- documentation that vestibular rehabilitation was recommended OR
- documentation that vestibular rehabilitation was provided

The denominator: patients diagnosed with unilateral or bilateral vestibular hypofunction
Exclusions are removed from the denominator:
- Patient has refused or declined vestibular rehabilitation services
- Patient previously received vestibular rehabilitation services and it was determined to not be effective
- Patient is clinically asymptomatic or compensated in unilateral or bilateral vestibular hypofunction

Measure Calculation Example
Dr. Bautista has 100 patients between January 1 and December 31 that were diagnosed with unilateral or bilateral vestibular hypofunction. This is the denominator.
Two patients refused or declined vestibular rehabilitation services. One patient previously received vestibular rehabilitation services that were ineffective in addressing symptoms and was not provided vestibular rehabilitation services as a result. Three patients were clinically asymptomatic. These six patients (2+1+3) are removed from the denominator as exclusions.
Of the 94 remaining patients, 38 received vestibular rehabilitation services and 17 were referred to physical therapy or occupational therapy for vestibular rehabilitation services. The numerator is 55 = (38+17).
Quality of patient care = (38+17)/(100-6). Dr. Bautista has a score of 57.3% for this measure.
