The numerator: Patients for whom a MD multi-disciplinary care plan* was developed, if not done previously, or the plan was updated at least once annually.

The denominator: All patients diagnosed with a muscular dystrophy
Exclusions are removed from the denominator for:
- Plan was updated with 12 months of the date of the encounter
- Patient or family caregiver declines
- Patient has no insurance to cover the cost of seeing specialists or other clinicians in a multi-discplinary care plan, cannot travel to see specialist, multi-disciplinary services unavailable

Measure Calculation Example
Dr. Ouellet had 30 patients with muscular dystrophy seen between January 1 and December 31 of the measurement year. This is the denominator. Three patients already had an updated plan and two declined. These patients are removed from the denominator.
Of the remaining 25 patients, 17 had multi-disciplinary care plans developed or updated. This is the numerator.
Quality of care = 17/(30-5). Dr. Ouellet has a score of 68% for this measure.
