Measure purpose: Routinely screen patients with multiple sclerosis (MS) for cognitive impairment
The numerator: Patients screened for cognitive impairment in the past 12 months, and if screening positive, patient was referred appropriately for further evaluation and management
is divided by
the denominator: All patients with a diagnosis of MS
Exclusions are removed from the denominator:
- Patient declines to complete a cognitive assessment on date of encounter
- Patient is not able to complete a cognitive assessment on date of encounter
- Patient currently receiving treatment to address cognitive impairment
- Patient not seen in past 12 months

Measure Calculation Example
Dr. Johnson sees 100 patients diagnosed with MS between January 1 and December 31. This is the denominator.
Four patients decline to participate in a cognitive assessment. Three patients were receiving treatment to address known cognitive impairments. These seven (4+3) patients are removed from the denominator as exclusions.
Of the remaining 93 patients, 17 patients had documentation that they were screened for cognitive impairment and no follow-up was needed and 7 patients had documentation that screening was completed and appropriate follow-up cooordinated in the past 12 months. This is the numerator.
Quality of patient care = (17+7)/93 (100-7). Dr. Johnson has a score of 25.8% for this measure.
