Exclusions are removed from the denominator for:
- Patient does not need therapy based upon MD phenotype
- Patient already currently being seen by physical therapy, occupational therapy, or speech/swallowing specialist
- Patient or caregiver decline
- Patient does not have insurance to cover therapy
- Patient cannot travel to the therapy facility

Measure Calculation Example
Dr. Baker had 30 patient visits with patients with muscular dystrophy from January 1 to December 31 of the measurement year. This is the denominator. Two patients do not need therapy due to MD phenotype, two are already in therapy, and one declines the referral. These patients are removed from the denominator.
Of the remaining 25 patients, one was referred to all three services, five were referred to physical therapy, six were referred to occupational therapy, and three were referred to speech/swallowing therapy. This is the numerator.
Quality of care = 15/(30-5). Dr. Baker has a score of 60% for this measure.
