Functional status assessment for patients with dementia
Measure Purpose: Identify functional status deficits
The numerator: Patients for whom an assessment of functional status* is performed and the results reviewed at least once within a 12-month period
is divided by
the denominator: all patients with dementia (see measure specification for complete list of qualifying diagnoses)
Exclusions are removed from the denominator for:
- Documentation of a medical reason for not assessing functional status

Measure Calculation Example
Dr. Okafor sees 100 patients between January 1 and December 31 with a diagnosis of dementia. This is the denominator.
Of the 100 patients, Dr. Okafor assesses functional status for 74 patients.
Quality of patient care = 74/100. Dr. Okafor has a score of 74% for this measure.
