Measure purpose: Ensure patients have a multidisciplinary plan developed and updated annually.
The numerator: visits during the calendar year during which a multidisciplinary care plan was either developed (if not done previously) or reviewed and/or updated
is divided by
The denominator: all outpatient and telehealth visits for patients with a primary diagnosis of ALS phenotypes characterized by appropriate ICD codes
Exceptions are removed from the denominator:
- Patient/caregiver declines multidisciplinary care plan.
- Patients identified as not in current need of multidisciplinary care planning with an early, nondebilitating form of ALS (e.g., King’s Staging System, Stage 1)

Measure Calculation Example
Between January 1 and December 31, Dr. Koothrappali sees, through either outpatient or telehealth visits, 123 patients with a primary diagnosis of ALS phenotypes characterized by appropriate ICD codes. This is the denominator.
Two patients decline a multidisciplinary care plan and 1 patient is identified as not in current need of multidisciplinary care planning. These exceptions are removed from the denominator (123-3=120).
Dr. Koothrappali develops or updates multidisciplinary care plans for 79 of these patients within the year. This is the numerator.
Quality of patient care = 79/123-3. Dr. Koothrappali has a score of 65.8% for this measure.
