Exclusions are removed from the denominator for:
- Patient cannot tolerate evaluation
- MD phenotype does not warrant evaluation
- Patient or family caregiver declines an evaluation
- Clinic does not have the necessary equipment
- Patient cannot travel for testing
- Patient does not have insurance coverage

Measure Calculation Example
Dr. Landry had 20 patients with muscular dystrophy between January 1 and December 31 of the measurement year. This is the denominator. One patient cannot tolerate evaluation, one does not have the appropriate MD phenotype, and two patients declined the evaluation. These patients are removed from the denominator.
Of the remaining 16 patients, four had a pulmonary status evaluation ordered. This is the numerator.
