Measure Purpose: To ensure the safety of patients by providing them with a plan for falling
The numerator: Patients with a plan of care* for falls documented (including plans created by another provider) in the measurement period.
*Plan of care must include consideration of balance, strength, and gait training OR a referral to physical therapy.

The denominator: Patients aged 18 and older with a neurological condition that reported a fall during the measurement period.
Exclusions:
- Patient is bedridden, immobile, not ambulatory
- No documentation of falls inquiry or discussion during patient visit

Measure Calculation Example
Dr. Mayer saw 100 patients over the age of 18 from January 1 to December 31 of the measurement year. Of those 100 patients, 40 reported they had a fall during the year. This is the denominator.
Of the 40 patients, 12 were given a plan of care for falls. This is the numerator.
Quality of care =12/40. Dr. Mayer has a score of 30% for this measure.
