Measure Purpose: Ensure patients with Parkinson's Disease (PD) are counseled on importance of and provided regular exercise regimen recommendations yearly.
The numerator: patients with PD counseled on importance of and provided recommendations on regular exercise regimen* in the past 12 months.
*Regular exercise regimen is defined as at least 150 minutes of moderate intensity activity each week per the Department of Health and Human Services (HHS)

The denominator: all patients with a diagnosis of PD.
